If you are used to buying men's sizes buy a size larger in the ladies. So if you usually buy her a men's medium you will need to buy her a ladies large.
Do they shrink? We have found that after washing these shirts there is no noticable shrinkage at the shoulders but they do shorten up by about 1/2 an inch at the hem.
